, Hinsdale, IL The role you'll contribute:
The Patient Care Manager is responsible for the management of multiple team(s) of clinical and support staff providing hospice and palliative care to an assigned case load of patients.
The position is responsible for the overall direction, coordination, and evaluation of all staff on their assigned team.

o Provides overall direction of the operations related to assigned interdisciplinary team(s).
o Maintains knowledge and communicates to staff regarding policies and procedures required to comply with federal, state and local guidelines as well as Medicare, Medicaid and insurance reimbursement requirements.
Interprets requirements to determine eligibility when a unique or questionable case is referred.
o Develops, implements, and evaluates interdisciplinary team (IDT) goals and objectives.
o Reviews IDT documentation and charting to ensure it is timely, accurate and complete.
Communicates with Health Information Management department to update incomplete documentation.
o Investigates, documents, resolves and reports on problems and issues involving staff or quality of care.
o Conducts weekly interdisciplinary team meetings to review and ensure patient care, staff relations, and efficiency of service.
o Acts as a liaison with physicians regarding clinical issues.
o Acts as a liaison with family members to address questions, resolve problems, or counsel regarding end of life expectations.
o Accompanies staff periodically to evaluate performance and knowledge and lend expertise and guidance to improve subordinate skills.
o Carries out supervisory responsibilities in accordance with the organization's policies, procedures and applicable laws, including employment laws.
o Works collaboratively with the Human Resources Department in accomplishing employee interviewing, hiring, training and coaching functions.
o Works collaboratively with the Human Resources Department to address complaints and assist in resolving employee relation issues.
o Rewards and holds employees accountable for their performance/behaviors as necessary.

Qualifications The expertise and experiences you'll need to succeed:
EDUCATION AND EXPERIENCE REQUIRED:
oMinimum of 3 years' experience as a Registered Nurse including three years supervisory experience.
oAn equivalent combination of education and experience may be substituted at the discretion of the Administrator.
EDUCATION AND EXPERIENCE PREFERRED:
oBachelor of Science in Nursing, Business Administration, Healthcare Management, or related field.
oHome health/Hospice and/or acute care nursing experience.
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
oAHA Basic Life Support Certification.
oMaintain a current, valid driver's License in good standing with a safe driving record and a reliable vehicle.
oMaintain valid personal automobile insurance.
LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:
